The Intake Application is what is needed to start the intake process. This is required to be completed before the intake interview is held. It gives our staff the information needed about the family and children's needs.
The Court Referral Form is for the Judges, Courts and Child Protective Service workers who refer families directly to The Green House Family Resource Center.
